Our program is designed to support patients living with diabetes through personalized care plans, ongoing monitoring, preventive screenings, and education. Each patient is paired with a dedicated case manager who coordinates care between your primary provider, specialists, and support services.
Most major insurance plans, including Medicare and many commercial payors, provide coverage for diabetes care management services. Coverage may include:
- Chronic Care Management (CCM)
- Remote Patient Monitoring (RPM)
- Preventive screenings and lab work
- Ongoing education and nutrition support
We recommend checking with your insurance provider to confirm coverage details, copayments, or deductibles.
Remote Patient Monitoring uses cellular based devices such as glucose monitors, blood pressure cuffs, or weight scales provided by us, that transmit data to our care team. This allows your provider to track trends, identify concerns early, and adjust your care plan without waiting for your next clinic visit. RPM improves outcomes by keeping your care team informed in real time.
CCM is a program that provides ongoing support for patients with chronic conditions such as diabetes, hypertension, or heart disease.
Through CCM you’ll receive:
- Monthly check-ins from your case manager
- A personalized care plan
- Coordination with specialists and community resources
This ensures your care is consistent and proactive.

These assessments guide your personalized care plan.
Yes! Every patient in the program is paired with a dedicated case manager who:
- Coordinates care between your primary provider, specialists, and hospital services
- Ensures timely follow-up on labs, medications, and preventive screenings
- Acts as your direct point of contact for questions or concerns
- RPM patients: Your care team monitors your readings daily and will reach out if there are concerns.
- CCM patients: You’ll receive at least one monthly check-in, with additional support as needed.
- Clinic visits: Preventive and follow-up visits are scheduled as recommended by your provider.
- A willingness to stay engaged with your care plan
- Regular communication with your case manager and care team

Enrollment is simple. Speak with your primary care provider at Family Hospital or call our care management team directly. We will verify your insurance, complete your initial assessment, and assign you a case manager.
